Indonesia's efforts in extending universal health coverage (UHC) have been markedly advanced by its National Health Insurance (NHI) system. In contrast to an envisioned universal access, Indonesia's NHI rollout confronted socioeconomic variations in comprehension of NHI concepts and processes across various population segments, thus amplifying potential inequalities in healthcare accessibility. In this study, the secondary data analysis was based on The Ministry of Health of the Republic of Indonesia's 2019 nationwide survey, which covered 'Abilities and Willingness to Pay, Fee, and Participant Satisfaction in implementing National Health Insurance in Indonesia'. Indonesia's impoverished community, represented by a weighted sample of 18,514 people, constituted the study population. In the study, NHI membership served as the dependent variable. In the study, seven independent variables—wealth, residence, age, gender, education, employment, and marital status—were examined. Analysis of the data reveals a pattern wherein NHI membership is notably higher among the impoverished demographic possessing higher educational levels, residing in urban environments, exceeding 17 years of age, being married, and having higher financial standing. NHI membership among the impoverished is disproportionately higher for those with higher educational levels compared to those with lower levels of education. In predicting their NHI membership, various factors were assessed, including their place of residence, age, gender, employment status, marital status, and financial situation. Poor individuals holding primary education are significantly, 1454 times more likely to become members of NHI, as compared to those devoid of any formal education (AOR = 1454; 95% CI: 1331–1588). The findings from our study, revealing substantial variations in predictive factors amongst the poor based on differing educational attainment, highlight the critical imperative for government investment in NHI, interwoven with investments in education for the impoverished.
NHI membership among the impoverished population is predictably correlated with factors such as educational attainment, place of residence, age, sex, employment status, marital standing, and economic standing. Categorizing and connecting physical activity (PA) with sedentary behavior (SB) is key to creating successful lifestyle interventions for the youth population. This systematic review (Prospero CRD42018094826) investigated the co-occurrence patterns of physical activity (PA) and sedentary behavior (SB), and their relationship to demographic factors, in boys and girls from 0 to 19 years of age. The investigation employed five electronic databases in its search. Independent reviewers, following the authors' delineations, extracted cluster characteristics, and any resulting disputes were resolved by a third reviewer. Individuals aged six to eighteen years were represented in seventeen studies that met the inclusion criteria. Mixed-sex samples exhibited nine, boys twelve, and girls ten distinct cluster types. Female clusters exhibited low physical activity with low social behavior, and low physical activity levels in conjunction with high social behavior. The majority of male clusters, however, were defined by high levels of physical activity and high social behavior, and high physical activity with low social behavior. Sociodemographic details demonstrated a paucity of associations with all the identified clusters. High PA High SB clusters presented elevated BMI and obesity levels in both boys and girls, across most examined associations. Differing from the other groups, those categorized in the High PA Low SB clusters displayed a lower BMI, waist circumference, and lower rates of overweight and obesity. Boys and girls showed contrasting clustering of PA and SB, a key finding in this study. Nevertheless, a more favorable adiposity profile emerged in children and adolescents categorized within the High PA Low SB clusters, regardless of sex. Since 2019, the reform of China's medical system inspired Beijing municipal hospitals to implement a novel pharmaceutical care model, setting up medication therapy management (MTM) services within their outpatient care. In China, our hospital was among the initial medical facilities to establish this service. At the present time, there were not many reports on the impact MTMs were having in China. This research investigates the implementation of MTMs in our hospital, explores the potential of pharmacist-led MTMs in ambulatory patient care, and assesses the influence of MTMs on patient medical expenses.
In Beijing, China, a university-connected, comprehensive tertiary hospital served as the site for this retrospective analysis. Patients documented with complete medical and pharmaceutical files, having received one or more Medication Therapy Management (MTM) services from May 2019 through February 2020, were part of the study group. In accordance with the American Pharmacists Association's MTM standards, pharmacists meticulously delivered pharmaceutical care to patients. Their responsibilities included cataloging patients' perceived medication demands by number and type, identifying medication-related problems (MRPs), and developing corresponding medication-related action plans (MAPs). A record was kept of every MRP identified by pharmacists, pharmaceutical intervention employed, and resolution recommendation made, in addition to calculating the decrease in cost for treatment drugs for patients.
Of the 112 patients who received MTMs in ambulatory care, 81, possessing complete records, were selected for inclusion in this research. Patients exhibiting five or more co-occurring diseases comprised 679% of the total. A considerable portion, 83%, of these patients also simultaneously took over five different medications. Complex care needs and a deficiency of nursing personnel pose challenges for healthcare professionals working in nursing homes. Subsequently, nursing homes are adapting to become personalized, home-style facilities focused on the individual. The evolving dynamics of nursing homes, and the challenges involved, necessitate the establishment of an interprofessional learning culture, but the precise elements that cultivate and support such a culture remain obscure. This scoping review is designed to uncover the key elements that facilitate the identification of these specific facilitators.
A scoping review was undertaken using the JBI Manual for Evidence Synthesis (2020) as the guiding document. The years 2020 and 2021 witnessed a search performed across seven global databases: PubMed, Cochrane Library, CINAHL, Medline, Embase, PsycINFO, and Web of Science. Reported facilitators of interprofessional learning cultures in nursing homes were independently documented by the two researchers. Using an inductive methodology, the researchers classified the gleaned facilitators into specific categories.
The comprehensive search unearthed 5747 studies. Thirteen studies were included in this scoping review; these studies met all the inclusion criteria after the removal of duplicates and the screening of titles, abstracts, and full texts. We grouped 40 facilitators into eight clusters: (1) common language, (2) common aims, (3) distinct responsibilities and duties, (4) knowledge exchange and learning, (5) collaborative procedures, (6) change facilitation and creative support by the front-line supervisor, (7) open-mindedness, and (8) a secure, respectful, and transparent setting.
Facilitators were utilized to explore and assess the current state of interprofessional learning within nursing homes, pinpointing necessary improvements.
